Roles & Responsibilities

Provide compassionate, resident-centered nursing care with professionalism and warmth
Administer medications and treatments accurately and document appropriately
Perform routine assessments and monitor resident conditions, escalating changes as needed
Collaborate with the interdisciplinary team to implement individualized care plans
Assist with admissions, transfers, and discharges while ensuring smooth transitions
Support residents with activities of daily living and encourage independence
Maintain accurate, timely documentation in the medical record
Participate in quality improvement and safety initiatives to enhance resident outcomes

Qualifications

Current LPN license in Connecticut (required)
Graduation from an accredited LPN program or equivalent clinical training
Basic Life Support (BLS) certification preferred or obtained upon hire
Comfortable working in long-term care or skilled nursing environments
Strong communication skills and ability to work well in a team
Reliable, compassionate, and organized with attention to detail

Education

Completion of an accredited LPN (Practical Nursing) program; high school diploma or equivalent required

Knowledge & Skills

Medication administration and documentation best practices
Resident assessment and reporting clinical changes
Wound care basics and dressing changes
Familiarity with electronic medical records and charting
Infection control and safety protocols
Time management and prioritization in a fast-paced care setting
