Roles & Responsibilities

Provide compassionate, direct patient care with skill and attention to detail
Administer medications and treatments per provider orders and facility policy
Perform accurate assessments and monitor patients for changes in condition
Document patient care thoroughly and timely in the medical record
Collaborate with nurses, providers, and the care team to deliver coordinated care
Educate patients and families about care plans and promote positive outcomes
Maintain safety and infection control standards to protect patients and staff

Qualifications

Current, active LPN license (eligible to practice in Pennsylvania preferred)
Recent clinical experience as an LPN or in a related nursing role is a plus
Basic Life Support (BLS) certification or willingness to obtain upon hire
Strong clinical judgment, communication skills, and ability to work in a team
Ability to pass facility background checks and meet onboarding requirements

Education

LPN certificate / Vocational Nursing diploma or equivalent

Knowledge & Skills

Medication administration and documentation best practices
Patient assessment and monitoring
Familiarity with electronic medical records (EMR) or charting systems
Infection control, safety protocols, and clinical policies
Good interpersonal skills and the ability to teach patients and families
