Roles & Responsibilities

• Provide safe, compassionate nursing care to residents/patients in line with facility policies
• Administer medications and treatments accurately and document thoroughly
• Perform routine assessments and report changes in condition to the care team
• Collaborate with RNs, CNAs and interdisciplinary staff to create and follow care plans
• Maintain clear, timely charting in the resident/patient record
• Support admissions, discharges and transfers with professionalism and attention to detail
• Promote a safe, clean environment and follow infection-control protocols
• Educate residents/patients and families about care plans and health needs

Qualifications

• Active LPN license in Pennsylvania (required)
• Graduate of an accredited Practical Nursing/LPN program
• BLS/CPR certification (current) — preferred or obtain upon hire
• Strong communication and interpersonal skills to connect with residents and team members
• Ability to perform duties reliably during scheduled shifts and maintain professional documentation
• Comfort working as part of a collaborative care team in a facility setting

Education

• Graduate of an accredited Practical Nursing / LPN program (diploma or certificate)

Knowledge & Skills

• Medication administration and safe handling
• Basic patient assessment and vital sign monitoring
• Familiarity with wound care and common long-term care procedures
• Accurate clinical documentation and electronic medical record (EMR) basics
• Infection control, resident safety, and fall prevention best practices
• Teamwork, time management, and patient-centered communication
