Roles & Responsibilities

Deliver compassionate, hands-on nursing care to residents in a skilled nursing environment
Administer medications and treatments accurately and document all care thoroughly
Monitor resident condition, report changes promptly, and collaborate with the care team
Perform routine assessments and support individualized care plans
Assist with admissions, transfers, and discharges while ensuring resident comfort and safety
Maintain accurate, timely charting in the patient record and follow facility policies
Participate in interdisciplinary team meetings to advocate for resident needs

Qualifications

Current and active LPN license in Connecticut (required)
Basic Life Support (BLS/CPR) certification (or obtain upon hire)
Comfortable working in long-term care / skilled nursing settings
Current immunizations and ability to pass required health screenings/background checks
Strong communication skills and a team-first attitude
Previous LPN experience in a similar setting is a plus

Education

Graduation from an accredited practical nursing program
Current LPN licensure in Connecticut

Knowledge & Skills

Medication administration and documentation best practices
Clinical assessment and basic nursing procedures
Infection control and safety protocols
Electronic health record (EHR) familiarity and accurate charting
Ability to prioritize tasks and manage time in a fast-paced setting
Collaborative teamwork and resident-centered communication
