Roles & Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans that support functional independence
• Provide targeted therapy focused on mobility, strength, balance, and fall prevention
• Monitor patient progress, adjust treatment plans as needed, and document outcomes clearly
• Collaborate with physicians, occupational therapists, nurses, and the interdisciplinary team to deliver coordinated care
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies to enhance daily living
• Advocate for residents' rehabilitation goals and contribute to quality improvement initiatives

Qualifications

• Active California Physical Therapist License (required)
• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program (required)
• Previous experience in Skilled Nursing Facility (SNF), rehabilitation, or geriatric therapy preferred
• Strong clinical assessment, treatment planning, and patient care skills
• Ability to work effectively within a multidisciplinary team and communicate clearly with patients and colleagues

Education

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program

Knowledge & Skills

• Expertise in mobility training and rehabilitation techniques
• Proficiency in patient evaluation and individualized treatment planning
• Strong documentation skills and familiarity with clinical records
• Effective communication with residents, families, and care teams
• Teamwork and a patient-centered approach to care
