Roles & Responsibilities

• Provide individualized, compassionate nursing care to skilled nursing facility residents
• Conduct thorough patient assessments and notify charge nurse/physician of abnormal findings
• Develop and execute individualized plans of care in collaboration with the healthcare team
• Administer medications and treatments accurately, following facility policy and documentation standards
• Perform clinical procedures such as IV starts, EKGs, and phlebotomy as ordered
• Initiate emergency resuscitation measures promptly and document interventions
• Maintain accurate, timely documentation and charts to ensure continuity of care
• Restock and organize supplies (IV room, wound cart) and keep patient areas clean and safe
• Assist with admissions, transfers, and discharge coordination in partnership with other departments
• Support orientation and education of new staff and participate in monthly staff meetings

Qualifications

• Active California LVN license required
• Current BLS/CPR certification required; maintain certification annually
• Proven skills in medication administration, IV starts, EKGs, and phlebotomy
• Strong patient assessment, clinical judgment, and emergency response abilities
• Good physical stamina and dexterity to perform patient care tasks (lifting, walking, bending)
• Excellent verbal and written English communication skills
• Dependable attendance, professionalism, and ability to follow chain-of-command
• Willingness to wear Caribbean Blue scrubs while on assignment (this is required)

Education

• Graduate of an accredited Vocational Nursing / LVN program (or equivalent) and active CA LVN license

• Nursing care and patient assessment best practices
• Medication administration and narcotics control procedures
• IV therapy, phlebotomy, and EKG performance
• Wound care and infection control standards
• Accurate documentation and charting practices
• Emergency resuscitation protocols and rapid response
• Comfort with Microsoft Office for documentation and reporting
• Time management, teamwork, and effective interpersonal communication
• Ability to use office machines and maintain organized patient records
• Patient/family education and culturally sensitive care
