Roles & Responsibilities

• Assess and monitor patient conditions to ensure safe, effective care
• Administer medications and treatments accurately and on schedule
• Collaborate with interdisciplinary rehab teams to develop and execute care plans
• Document care, progress, and changes meticulously in the medical record
• Provide patient and family education to support recovery and independence
• Support safe patient mobility and rehabilitation activities during shifts
• Communicate clearly with physicians and team members to coordinate care
• Champion patient safety, dignity, and a positive healing environment

Qualifications

• Active Registered Nurse or Licensed Practical Nurse licensure (RN or LPN) as indicated by the job class
• Available and able to work the listed day shift hours (6:30 AM - 7:00 PM)
• Comfortable working in a rehab/long-term care environment
• Travel/contract-ready with flexibility for assignment logistics

Knowledge & Skills

• Patient assessment and clinical observation skills
• Medication administration and documentation best practices
• Strong communication and teamwork in interdisciplinary settings
• Familiarity with rehab-focused care and mobility assistance
• Time management and prioritization during extended day shifts
• Compassionate patient- and family-centered care approach
