Shift Type: Rotating night (NOC) schedule — 6:00 PM - 6:00 AM, 12-hour NOC shifts, 36 hours/week (specific days vary); every-other-weekend rotation and every-other-holiday requirement; must be able to float to vents, rehab, long-term care and dementia units

Roles & Responsibilities

• Provide expert ventilator and tracheostomy care to ventilator-dependent residents
• Perform suctioning safely and efficiently to maintain airway patency
• Administer and monitor tube feedings and manage wound care with attention to detail
• Monitor clinical status, recognize changes, and intervene appropriately
• Float between vents, rehab, long-term care and dementia units to meet resident needs
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Maintain accurate documentation and adhere to infection control and facility policies
• Participate in weekend and holiday rotations as scheduled

Qualifications

• Active Wisconsin RN license required at time of submission
• Active BLS certification required
• Ventilator and tracheostomy care experience is a MUST
• Experience with suctioning, tube feedings and wound care
• Comfort working in skilled nursing, post-acute rehab and dementia care settings
• Willingness to float to multiple units and work every-other-weekend/holiday rotations
• Reliable, professional, and able to work a fixed night schedule (no schedule restrictions)

• Ventilator management and tracheostomy care
• Advanced suctioning techniques
• Tube feeding protocols and enteral nutrition monitoring
• Wound assessment and dressing management
• Care for residents in long-term care, rehab and dementia units
• Ability to manage assigned resident ratios and prioritize workload
• Strong clinical judgment, communication and teamwork
