Roles & Responsibilities

Provide compassionate, patient-centered nursing care consistent with LPN scope of practice
Administer medications and treatments accurately and document per facility policy
Monitor patient conditions and report changes to the care team promptly
Assist with daily living activities and support patients with dignity and respect
Collaborate with RNs, physicians and interdisciplinary staff to deliver excellent care
Maintain accurate charting and ensure regulatory and safety standards are met

Qualifications

Current, active LPN license in Pennsylvania (required)
Basic Life Support (BLS/CPR) certification or willingness to obtain
Comfortable working in long-term care/acute care settings and supporting a diverse patient population
Strong communication skills and ability to work well within a team
Previous clinical experience preferred but new graduates with clinical training are welcome to apply

Education

LPN diploma or equivalent vocational nursing program completion
High school diploma or GED

Knowledge & Skills

Medication administration and documentation best practices
Fundamentals of patient assessment and vital signs monitoring
Wound care basics and infection control procedures
Charting/EHR familiarity and accurate record-keeping
Effective communication with patients, families and clinical teams
