Roles & Responsibilities

• Deliver compassionate, hands-on nursing care to residents/patients as directed
• Administer medications and treatments accurately and document care thoroughly
• Monitor residents’ conditions and report changes promptly to the care team
• Collaborate with RNs, physicians, and interdisciplinary staff to support care plans
• Assist with admissions, transfers, and discharges while maintaining resident dignity
• Maintain clear, timely charting and ensure compliance with facility policies
• Support a positive, safety-first environment for residents and team members

Qualifications

• Current LPN (Licensed Practical Nurse) license in Pennsylvania required
• Completion of an accredited practical nursing program (diploma or equivalent)
• Current BLS/CPR certification preferred or willingness to obtain
• Ability to pass required background checks and health screenings
• Strong communication skills and a collaborative, team-oriented attitude
• Previous LPN experience preferred but new grads with the right training are welcome to apply

Education

• Practical Nursing diploma or equivalent (completion of an accredited LPN program)

Knowledge & Skills

• Medication administration and basic wound care
• Patient assessment and vital signs monitoring
• Electronic and paper charting proficiency
• Infection control and safety protocols
• Time management, prioritization, and dependable clinical judgment
• Compassionate bedside manner and effective teamwork
