Roles & Responsibilities

Provide compassionate direct patient care in accordance with facility policies
Administer medications and treatments as ordered and document appropriately
Monitor and document patient status, reporting changes to the care team
Assist with admissions, discharges, and transitions of care
Collaborate with RNs and interdisciplinary team members to deliver safe care
Maintain accurate clinical records and ensure medication administration records are up to date
Follow infection control, safety, and compliance protocols at all times

Qualifications

Active LPN license in Pennsylvania (required)
Completion of an approved LPN training program or diploma
Ability to work 8-hour shifts as scheduled by the facility
Current Basic Life Support (BLS) certification preferred or as required by facility
Clear background checks and any facility-specific onboarding requirements

Education

Licensed Practical Nurse (LPN) diploma or certificate

Knowledge & Skills

Patient assessment and basic clinical skills
Medication administration and documentation
Basic wound care and vital sign monitoring
Electronic medical record (EMR) charting proficiency or willingness to learn
Strong communication and teamwork skills
Time management and ability to prioritize patient care
