Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Administer medications and treatments accurately and document thoroughly
- Perform regular patient assessments and report changes to the care team
- Support care planning and coordinate with nurses and clinicians for continuity
- Maintain clear, respectful communication with residents, families, and staff
- Follow facility protocols for safety, infection control, and documentation
Qualifications
- Current LPN (Licensed Practical Nurse) license valid in Pennsylvania
- BLS/CPR certification or willingness to obtain upon hire
- Up-to-date immunizations and ability to pass background/health screenings
- Strong interpersonal skills and a patient-centered approach
- Comfortable with electronic health record documentation
Education
- LPN diploma or vocational nursing certificate and required state licensure
Knowledge & Skills
- Medication administration and basic IV/clinical skills relevant to LPN scope
- Wound care, vital signs, and standard nursing assessments
- Familiarity with long-term care or skilled nursing workflows is a plus
- Good charting practices and attention to detail
- Ability to prioritize tasks and work effectively as part of a team
