Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents in accordance with care plans
- Administer medications and treatments accurately and document thoroughly
- Perform timely patient assessments and report changes to the interdisciplinary team
- Assist with activities of daily living while promoting resident dignity and independence
- Collaborate with nurses, providers, and support staff to deliver coordinated care
- Maintain accurate charting and uphold compliance with facility policies and regulatory standards
- Support a safe, clean environment and participate in infection control practices
Qualifications
- Current, active LPN license for Pennsylvania
- CPR/BLS certification (or willingness to obtain upon hire)
- Ability to complete required facility onboarding and background checks
- Strong communication skills and a team-oriented attitude
- Comfortable with medication administration, basic wound care, and documentation
Education
- Graduate of an accredited practical nursing program or equivalent vocational training
Knowledge & Skills
- Medication administration and monitoring
- Basic nursing assessments and vital signs
- Charting and electronic health record familiarity
- Infection control practices and safe patient handling
- Effective communication with residents, families, and clinical team
