Roles & Responsibilities
- Provide compassionate, competent nursing care to residents as an LPN, following care plans and facility policies
- Administer medications and treatments safely and document accurately
- Monitor resident conditions and communicate changes promptly to the care team
- Support activities of daily living and promote resident dignity and independence
- Collaborate with nurses, therapists, and family members to deliver personalized care
- Participate in charting, resident assessments, and handoffs to ensure continuity of care
Qualifications
- Valid Licensed Practical Nurse (LPN) license in Pennsylvania (required)
- Current BLS/CPR certification (or willing to obtain upon hire)
- Comfortable with medication administration, wound care basics, and documentation
- Strong communication skills and a team-first attitude
- Previous long-term care or skilled nursing experience preferred (1–2 years preferred)
Education
- Graduate of an accredited LPN/Licensing Practical Nurse program
Knowledge & Skills
- Knowledge of basic nursing procedures, infection control, and safe medication administration
- Skilled in resident assessment, reporting changes, and following care plans
- Familiarity with electronic or paper charting and accurate documentation
- Ability to work collaboratively with interdisciplinary teams
- Compassionate bedside manner and strong patient advocacy skills
