Roles & Responsibilities

• Provide compassionate, direct patient care to residents in the Uptown Health/Rehab area
• Administer medications and treatments accurately and document thoroughly
• Collaborate with interdisciplinary teams to create and follow individualized care plans
• Monitor patient status and communicate changes promptly to the care team
• Support patients and families with education, empathy, and clear communication
• Maintain accurate electronic health records and ensure compliance with facility policies

Qualifications

• Active Licensed Practical Nurse (LPN) OR Registered Nurse (RN) license (eligible for practice in New Mexico)
• Current Basic Life Support (BLS) certification; ACLS or other specialty certs a plus
• Comfortable with medication administration, wound care, and resident assessments
• Excellent communication skills and a collaborative mindset
• Ability to pass facility credentialing and meet onboarding requirements

Education

• Graduate of an accredited LPN or RN nursing program (practical/vocational nursing or associate/BSN for RNs)

Knowledge & Skills

• Strong patient assessment and clinical decision-making
• Medication administration and IV/po skills as applicable
• Familiarity with charting and electronic health record systems
• Infection control best practices and safety-focused mindset
• Ability to work effectively within a rehab/long-term care environment
