Roles & Responsibilities

• Provide compassionate, resident-centered nursing care at the bedside
• Administer medications and treatments safely and document accurately
• Perform regular patient assessments and promptly report changes in condition
• Collaborate with RNs and the interdisciplinary team to support care plans
• Maintain clear, timely electronic documentation and participate in shift handoffs
• Support activities of daily living and encourage resident independence and dignity

Qualifications

• Current, active LPN (Licensed Practical Nurse) license (required)
• Comfortable working 12-hour night shifts and participating in a rotating weekend schedule
• Basic clinical skills in medication administration, patient assessment, and wound care
• Strong communication skills and ability to collaborate with nursing staff and care team
• Previous experience in long-term care, skilled nursing, or similar settings is a plus
• Willingness to complete facility-required training and certifications

Education

• Graduate of an accredited Practical Nursing program or equivalent LPN training/certificate

• Patient assessment and basic nursing interventions
• Safe medication administration and documentation practices
• Infection prevention and control procedures
• Electronic health record documentation
• Prioritization and time management during busy 12-hour shifts
