Roles & Responsibilities

Provide compassionate direct patient care and maintain dignity for residents
Administer medications and treatments accurately and safely
Document patient status, interventions, and responses in the medical record
Collaborate with nursing staff and interdisciplinary team to develop care plans
Monitor vital signs and report changes promptly to ensure resident safety
Support residents with daily living activities and promote a comfortable environment

Qualifications

Valid LPN license in Pennsylvania (required)
Current BLS/CPR certification preferred or willingness to obtain
Comfortable with medication administration, basic assessments, and documentation
Strong communication skills and a team-oriented mindset
Reliable, caring, and committed to quality resident care

Education

LPN diploma or certificate from an accredited practical nursing program

Knowledge & Skills

Skilled in basic nursing procedures and medication administration
Familiarity with long term care workflows and documentation
Ability to prioritize tasks and respond to changing patient needs
Good interpersonal skills for working with residents, families, and staff
Attention to detail and strong time management
