Roles & Responsibilities

• Provide compassionate direct patient care to residents, ensuring dignity and comfort
• Administer medications and treatments accurately and on schedule
• Monitor vital signs and clinical status, promptly reporting significant changes
• Document care and observations clearly in the medical record
• Collaborate with nurses, therapists, and interdisciplinary team members to coordinate care
• Assist with activities of daily living and promote patient independence when possible
• Support infection control practices and adhere to facility policies and procedures

Qualifications

• Current Licensed Practical Nurse (LPN) license — required
• BLS/CPR certification preferred or willing to obtain
• Prior experience in long-term care, rehab, or med-surg preferred but new grads with strong clinical skills are welcome to apply
• Strong communication skills and the ability to work well in a team

Education

• Completion of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

• Strong fundamentals in medication administration and basic nursing procedures
• Skilled at resident assessment, documentation, and care planning
• Comfortable with wound care, mobility assistance, and ADL support
• Ability to prioritize, multitask, and manage time during busy overnight shifts
• Good interpersonal skills for working with residents, families, and the care team
