Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and document treatments accurately and on time
• Monitor and record patient vital signs, reporting changes to the care team
• Collaborate with RNs and interdisciplinary staff to support patient goals
• Maintain clean, safe, and well-documented patient records in facility systems
• Support admissions, discharges, and transfers with professionalism and empathy
• Engage patients and families with clear communication and patient-centered care

Qualifications

• Current Licensed Practical Nurse (LPN) credential — required
• Ability to work the scheduled evening shift: Monday–Friday, 3:00 PM–11:30 PM
• Local candidates preferred (note: listing specifies LOCAL ONLY)
• Active, unrestricted license to practice as an LPN (bring documentation)
• Reliable, professional, and able to collaborate effectively with a care team
• Preferred: prior experience in a similar facility setting or with geriatric care

Education

• Graduate of an accredited practical nursing program (LPN)

Knowledge & Skills

• Strong clinical assessment and basic nursing skills
• Medication administration and safe documentation practices
• Patient safety and infection control awareness
• Clear verbal and written communication with patients and colleagues
• Time management and ability to prioritize tasks during busy shifts
• Comfort using electronic clinical documentation systems
