Roles & Responsibilities

• Provide compassionate assistance with activities of daily living (bathing, dressing, grooming, toileting) to promote resident dignity
• Monitor and record vital signs and report changes in condition to the nursing team promptly
• Follow individualized care plans and assist in restorative care activities
• Support safe mobility and transfers using proper body mechanics and assistive devices
• Maintain infection control practices, state vaccination requirements, and a safe environment for residents
• Document resident care accurately and communicate effectively with teammates and families
• Help create a warm, home-like atmosphere and build trusting relationships with residents

Qualifications

• Current, unrestricted Colorado CNA certification (required)
• Long Term Care (LTC) / Skilled Nursing Facility (SNF) experience required
• COVID vaccination in accordance with state mandate required
• Experience in travel CNA assignments and shared housing preferred
• Willingness to reside in furnished shared housing (no pets, no smoking) or provide your own housing
• Reliable, compassionate, team-oriented attitude and strong communication skills

Education

• High school diploma or GED preferred
• Current Colorado CNA certification required

Knowledge & Skills

• Solid knowledge of ADLs and resident-centered long-term care
• Infection control and safe patient handling practices
• Basic vital sign measurement and recognition of changes in condition
• Accurate documentation and reporting skills
• Effective interpersonal skills for working with residents, families, and multidisciplinary teams
• Time management and ability to prioritize resident needs
