Roles & Responsibilities
- Assess and evaluate patients to create meaningful, individualized treatment plans
- Perform thorough therapeutic assessments for neuromuscular, orthopedic, pulmonary and cardiovascular conditions
- Deliver hands-on and evidence-based therapeutic interventions to restore function
- Monitor treatment effects and recommend adjustments to physicians and the care team
- Document patient progress clearly and promptly to support continuity of care
- Collaborate with a multi-disciplinary team to ensure patient-centered outcomes
Qualifications
- Current Colorado Physical Therapy license (required)
- Basic Life Support (AHA) certification (required)
- Comfortable working in a rural community and willing to relocate to the San Luis Valley
- Strong clinical assessment and therapeutic intervention skills
- Excellent documentation and communication skills; team-oriented mindset

- Patient assessment and treatment planning
- Therapeutic interventions for neuromuscular and orthopedic conditions
- Pulmonary and cardiovascular rehabilitation approaches
- Integumentary care and wound-related considerations
- Clinical documentation and outcome tracking
