Roles & Responsibilities

• Evaluate patients across inpatient, swing bed, outpatient, and skilled nursing settings to determine functional status and therapy needs
• Develop individualized treatment plans based on physician orders, best practices, and patient goals
• Provide skilled therapeutic interventions that promote mobility, independence, and safety
• Document assessments, progress, and discharge plans accurately and timely in the EMR
• Collaborate with physicians, nurses, and the care team to coordinate comprehensive patient care
• Educate patients and families on treatment plans, home exercise programs, and safe mobility strategies
• Use clinical reasoning to adapt interventions for diverse diagnoses and complex cases
• Maintain compliance with California and federal PT regulations and facility policies

Qualifications

• Active California Physical Therapy License in good standing
• Successful completion of the National Physical Therapy Examination (NPTE)
• Current AHA Basic Life Support (BLS) certification
• Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited program
• 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
• Proficiency with EMR systems and basic computer skills
• Strong clinical reasoning, communication, and documentation skills

Education

• Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited program

Knowledge & Skills

• Patient evaluation and treatment planning across multiple care settings
• Skilled nursing and inpatient/outpatient therapy practices
• Accurate clinical documentation and familiarity with EMR workflows
• Excellent communication and interdisciplinary collaboration
• Problem-solving and high attention to detail
• Ability to manage diverse patient populations and complex diagnoses
