Roles & Responsibilities
- Evaluate patients across inpatient, swing bed, outpatient, and skilled nursing settings to determine functional status and therapy needs
- Develop individualized treatment plans based on physician orders, best practices, and patient goals
- Provide skilled therapeutic interventions that promote mobility, independence, and safety
- Document assessments, progress, and discharge plans accurately and timely in the EMR
- Collaborate with physicians, nurses, and the care team to coordinate comprehensive patient care
- Educate patients and families on treatment plans, home exercise programs, and safe mobility strategies
- Use clinical reasoning to adapt interventions for diverse diagnoses and complex cases
- Maintain compliance with California and federal PT regulations and facility policies
Qualifications
- Active California Physical Therapy License in good standing
- Successful completion of the National Physical Therapy Examination (NPTE)
- Current AHA Basic Life Support (BLS) certification
- Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited program
- 1+ year experience in acute care, swing bed/skilled nursing, or outpatient settings
- Proficiency with EMR systems and basic computer skills
- Strong clinical reasoning, communication, and documentation skills
Education
- Bachelor’s, Master’s, or Doctor of Physical Therapy (DPT) from an accredited program
Knowledge & Skills
- Patient evaluation and treatment planning across multiple care settings
- Skilled nursing and inpatient/outpatient therapy practices
- Accurate clinical documentation and familiarity with EMR workflows
- Excellent communication and interdisciplinary collaboration
- Problem-solving and high attention to detail
- Ability to manage diverse patient populations and complex diagnoses
- Commitment to ongoing professional development and evidence-based practice
