Roles & Responsibilities

• Assess and treat patients to improve mobility, manage pain, and reduce fall risk with compassion and clinical skill
• Develop and implement individualized treatment plans and help guide patients toward safe discharge
• Document evaluations, progress notes, discharges, daily notes, and insurance communications accurately and timely
• Communicate effectively with physicians, rehab staff, insurance companies, and patients to coordinate care
• Provide mobility training and functional task practice to enhance patient independence
• Support clinic operations by maintaining clean treatment areas and assisting with laundry and facility needs
• Participate in team meetings and perform other duties as requested to keep patient care seamless

Qualifications

• Experience in physical therapy practices such as pain management, mobility training, and fall risk assessment
• Strong documentation and clinical communication skills, including interaction with insurance processes
• Ability to create treatment plans, perform patient assessments, and track progress toward goals
• Comfort working collaboratively with a multi-disciplinary team and engaging patients with empathy
