Roles & Responsibilities
- Assess patients' physical abilities through thorough evaluations to develop personalized treatment plans
- Design and implement effective therapeutic interventions to improve strength, mobility, and function
- Educate patients and families on safe movement techniques, exercises, and home modifications
- Collaborate with occupational therapists, nurses, physicians, and the rest of the care team to coordinate outstanding care
- Monitor patient progress and adjust treatment plans to optimize outcomes
- Maintain accurate, timely documentation of assessments, progress notes, and treatment plans in line with facility policies
- Ensure care plans meet regulatory requirements and best practices in physical therapy
Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license in the state of practice (California licensure expected or ability to obtain)
- Experience in a skilled nursing facility or similar rehabilitation setting preferred
- Strong communication and interpersonal skills to engage patients and collaborate with the team
- Demonstrated ability to work independently while contributing to a multidisciplinary team
- Compassionate, patient-focused approach with solid problem-solving skills
Education
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
Knowledge & Skills
- Patient evaluation and development of individualized treatment plans
- Hands-on physical therapy interventions to improve mobility and function
- Knowledge of therapeutic modalities and adaptive equipment
- Skilled Nursing Facility care processes and care coordination
- Accurate clinical documentation and progress tracking
- Patient and family education techniques
- Interdisciplinary collaboration, communication, and teamwork
