Roles & Responsibilities

• Assess patients' physical abilities through thorough evaluations to develop personalized treatment plans
• Design and implement effective therapeutic interventions to improve strength, mobility, and function
• Educate patients and families on safe movement techniques, exercises, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and the rest of the care team to coordinate outstanding care
• Monitor patient progress and adjust treatment plans to optimize outcomes
• Maintain accurate, timely documentation of assessments, progress notes, and treatment plans in line with facility policies
• Ensure care plans meet regulatory requirements and best practices in physical therapy

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the state of practice (California licensure expected or ability to obtain)
• Experience in a skilled nursing facility or similar rehabilitation setting preferred
• Strong communication and interpersonal skills to engage patients and collaborate with the team
• Demonstrated ability to work independently while contributing to a multidisciplinary team
• Compassionate, patient-focused approach with solid problem-solving skills

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program

Knowledge & Skills

• Patient evaluation and development of individualized treatment plans
• Hands-on physical therapy interventions to improve mobility and function
• Knowledge of therapeutic modalities and adaptive equipment
• Skilled Nursing Facility care processes and care coordination
• Accurate clinical documentation and progress tracking
• Patient and family education techniques
