Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to help them regain function
• Develop and implement individualized treatment plans that target meaningful outcomes
• Document care accurately and timely in accordance with facility and regulatory standards
• Collaborate with interdisciplinary teams to coordinate seamless resident care
• Participate in care conferences and contribute to continuous quality improvement efforts
• Communicate effectively with residents, families, and staff to support person-centered goals

Qualifications

• Active California Physical Therapist license (required)
• Skilled nursing or post-acute care experience preferred
• Strong clinical reasoning and hands-on treatment skills
• Excellent verbal and written communication skills
• Dependable, flexible, and team-oriented approach to patient care
• Willingness to work either full-time staff roles or travel/contract assignments

• Patient evaluation and evidence-based treatment techniques
