Roles & Responsibilities

• Assess patients’ functional mobility and design individualized treatment plans that produce measurable progress
• Deliver evidence-based therapeutic interventions including therapeutic exercise, manual therapy, and gait training
• Collaborate with nurses, physicians and interdisciplinary team members to coordinate care and support safe discharges
• Document patient evaluations, progress notes and discharge summaries clearly and promptly in the electronic health record
• Educate patients and families on home programs, safety strategies, and ways to promote independence
• Monitor outcomes and modify treatment plans based on patient response and goals
• Promote a culture of safety, infection control and patient-centered care at all times

Qualifications

• Active Pennsylvania physical therapist license (required)
• Graduation from an accredited physical therapy program (DPT preferred)
• Current BLS/CPR certification
• Experience in inpatient rehabilitation, post-acute care, or outpatient settings preferred
• Strong clinical reasoning, documentation skills and effective communication with interdisciplinary teams
• Ability to manage a full caseload and prioritize patient safety

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy acceptable if meeting state licensure requirements

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise and functional mobility training
• Knowledge of common rehab diagnoses and post-operative protocols
• Proficient with documentation and charting in electronic health records
• Comfortable with safe patient handling techniques and mobility equipment
• Excellent interpersonal skills for patient education and team collaboration
• Ability to adapt treatment plans based on patient progress and barriers
