Roles & Responsibilities
- Screen, evaluate and re-evaluate inpatients and outpatients referred by a licensed physician
- Provide evidence-based physical therapy services across the lifespan, from pediatric to adult
- Develop and implement individualized treatment plans and document patient progress
- Coordinate care and communicate effectively with the interdisciplinary healthcare team and patients
- Perform quality assessment activities and maintain accurate patient-related documentation
- Contribute to a positive, team-oriented rehab environment and support patient-centered outcomes
Qualifications
- BS in Physical Therapy from an accredited physical therapy program (required)
- Current New Hampshire Physical Therapy license (required)
- BLS (AHA) certification (required)
- At least 1 year of clinical experience preferred but not required — new grads are welcome to apply
- Strong interpersonal skills and the ability to function well on a multi-disciplinary team
- Resume required; references preferred
Education
- BS in Physical Therapy (from an accredited program)
Knowledge & Skills
- Patient assessment and evaluation across pediatric to adult populations
- Treatment planning, therapeutic interventions and outcome-driven care
- Thorough patient documentation and quality assessment practices
- Care coordination and clear communication with patients and clinical teams
- Familiarity with EPIC or similar electronic medical records is a plus
- Comfort working in both inpatient and outpatient rehab settings
