Roles & Responsibilities

• Carry out treatment plans established by the supervising PT to help patients regain mobility and independence
• Provide therapeutic exercises, gait training, balance activities, and appropriate modalities
• Observe, document, and report patient performance, progress, and concerns to the PT
• Educate patients and families on therapy goals and home exercise programs to support recovery outside the clinic
• Maintain accurate, compliant documentation in line with state, federal, and company guidelines
• Collaborate with interdisciplinary staff to support patient-centered outcomes in the skilled nursing setting

Qualifications

• Active and current Physical Therapist Assistant (PTA) license in California (required)
• Graduate of an accredited Physical Therapist Assistant program
• Previous experience working in a skilled nursing facility (SNF) is required
• Strong clinical and communication skills with the ability to follow established treatment plans
• Competent in documentation and regulatory compliance related to therapy services
• Comfortable working under the supervision of a licensed PT and as part of a care team

Education

• Graduate of an accredited Physical Therapist Assistant program

Knowledge & Skills

• Hands-on knowledge of therapeutic exercise techniques and gait training
• Familiarity with balance training and common physical modalities
• Skill in patient and family education for home exercise programs
• Ability to assess and report patient progress clearly and professionally
• Good time management and documentation accuracy
• Team-oriented communication and collaboration skills
