Roles & Responsibilities
- Evaluate patients to assess mobility, strength, functional status and rehabilitation needs
- Develop and implement individualized treatment plans aligned with patient goals
- Deliver hands-on therapy, therapeutic exercises, and appropriate modalities to improve outcomes
- Monitor, document and communicate patient progress while ensuring regulatory compliance
- Educate patients and families on therapy goals, techniques, and home exercise programs
- Collaborate with nurses, physicians, OTs, SLPs and others to coordinate comprehensive care
- Maintain accurate records and timely documentation according to company and regulatory standards
Qualifications
- Active and current Physical Therapist (PT) license in California (required)
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Previous experience in a Skilled Nursing Facility (SNF) is required
- Strong clinical knowledge with solid problem-solving and patient care skills
- Excellent communication, organization, and teamwork abilities
Education
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
Knowledge & Skills
- Skilled in patient evaluation and functional assessment
- Proficient with developing and implementing individualized treatment plans
- Hands-on therapy techniques and therapeutic exercise delivery
- Accurate clinical documentation and familiarity with regulatory requirements
- Patient and family education for home exercise programs and recovery
- Ability to collaborate effectively with interdisciplinary healthcare teams
