Roles & Responsibilities

• Perform comprehensive patient evaluations to assess mobility, strength, and rehabilitation needs
• Develop and implement individualized treatment plans focused on restoring function and improving quality of life
• Provide hands-on therapy interventions, therapeutic exercises, and appropriate modalities
• Monitor and document patient progress while ensuring compliance with regulatory and company standards
• Educate patients and families on therapy goals, techniques, and home exercise programs
• Collaborate with nurses, physicians, occupational and speech therapists to coordinate patient care
• Maintain accurate, timely documentation that supports continuity of care

Qualifications

• Active and current Physical Therapist (PT) license in California
• Doctor of Physical Therapy (DPT) or equivalent from an accredited program
• Previous experience in a Skilled Nursing Facility (SNF) required
• Strong clinical knowledge with excellent problem-solving and patient care skills
• Excellent communication, organization, and teamwork abilities
• Ability to maintain regulatory-compliant documentation

Education

• Graduate of an accredited Physical Therapy program (Doctor of Physical Therapy - DPT or equivalent)
