Shift Type: PRN / Per Diem — flexible schedule (shifts vary; may include weekdays and weekends; exact times assigned based on facility needs)
Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Perform thorough patient evaluations to assess mobility, strength, and rehabilitation needs
- Develop and implement individualized treatment plans focused on patient goals and safe recovery
- Deliver hands-on therapy interventions, therapeutic exercises, and appropriate modalities
- Monitor and document patient progress accurately and in a timely manner to meet regulatory standards
- Educate patients and families on therapy goals, techniques, and home exercise programs to support continuity of care
- Collaborate closely with nurses, physicians, OTs, SLPs and other team members to ensure coordinated, high-quality care
- Maintain compliant, organized clinical records that reflect professional standards and company policy
Qualifications
- Active and current Physical Therapist (PT) license in California (required)
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Previous experience working in a Skilled Nursing Facility (SNF) required
- Strong clinical reasoning and problem-solving abilities
- Excellent communication skills for patient education and team collaboration
- Good organizational skills and attention to documentation detail
- Ability to work independently on PRN shifts and adapt to changing patient needs
Education
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
Knowledge & Skills
- Solid knowledge of rehabilitation principles, therapeutic exercise and mobility training
- Skilled in patient assessment, treatment planning and outcome measurement
- Comfortable using common therapy modalities and hands-on techniques
- Able to educate patients and caregivers effectively and compassionately
- Strong teamwork and interdisciplinary communication skills
- Proficient documentation practices aligned with regulatory and company standards
