Roles & Responsibilities
- Perform thorough patient evaluations to assess function, mobility, strength, and rehabilitation needs
- Develop and implement individualized treatment plans focused on patient goals and safety
- Deliver hands-on therapy, therapeutic exercises, and appropriate modalities to maximize outcomes
- Monitor and document patient progress accurately and timely, maintaining regulatory compliance
- Educate patients and families on therapy goals, techniques, and home exercise programs
- Collaborate with interdisciplinary team members to coordinate comprehensive patient care
Qualifications
- Active and current Physical Therapist (PT) license in California
- Graduate of an accredited Physical Therapy program (DPT or equivalent)
- Previous experience in a Skilled Nursing Facility (SNF) required
- Strong clinical knowledge with excellent problem-solving and patient care skills
- Excellent communication, organization, and teamwork abilities
- Comfortable with timely documentation and adherence to regulatory standards
- Willingness to work PRN/per-diem shifts as needed by the facility
Education
- Doctor of Physical Therapy (DPT) or equivalent from an accredited program
Knowledge & Skills
- Physical Therapy best practices and patient evaluation techniques
- Treatment planning and implementation tailored to individual needs
- Hands-on therapeutic interventions and therapeutic exercise prescription
- Patient education methods and home exercise program development
- Clinical problem-solving and outcome-focused care
- Strong interpersonal skills for teamwork and family education
- Accurate documentation practices and regulatory compliance awareness
