Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families in collaboration with the multidisciplinary team
• Utilize the nursing process to assess, plan, implement and evaluate individualized patient care plans
• Administer prescribed medications and treatments following approved techniques, policies, and state practice acts
• Document care and patient education clearly and according to standards of care
• Act as a clinical resource and patient advocate; escalate safety or experience concerns through the proper chain of command
• Participate in department performance improvement activities and shared governance initiatives
• Develop, initiate and execute patient and family instruction materials and education programs
• Float to other units as needed and provide clinical guidance to support longitudinal patient care

Qualifications

• Licensed as an RN in Illinois and Wisconsin (required)
• CDCES (Certified Diabetes Care and Education Specialist) certification required
• Approved CPR certification required within 90 days of hire
• Graduate of an accredited nursing program (BSN preferred)
• Demonstrated ability to collaborate effectively with interdisciplinary teams
• Comfort with floating to various units and working occasional variable shifts, weekends or on-call

Education

• Graduate of an accredited nursing program
• BSN preferred

Knowledge & Skills

• Strong knowledge of the nursing process and patient care planning
• Expertise in diabetes education and medication administration
• Excellent patient and family education skills with culturally sensitive approach
• Solid documentation practices and understanding of standards of care
• Critical thinking focused on patient safety and experience
• Experience with interdisciplinary collaboration and performance improvement activities
