Roles & Responsibilities

• Perform thorough patient evaluations to identify functional limitations and design individualized treatment plans
• Deliver targeted physical therapy interventions to improve mobility, strength, and independence
• Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and the interdisciplinary team to coordinate care
• Monitor patient progress and modify treatment plans to ensure optimal outcomes
• Maintain accurate, timely documentation of assessments, progress notes, and care plans in accordance with facility policies
• Ensure therapy interventions and care plans meet regulatory requirements and best-practice standards

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in the state of California (or ability to obtain one)
• Experience working in a skilled nursing facility (SNF) or similar healthcare setting preferred
• Comfortable with therapeutic modalities, adaptive equipment, and treatment plan development
• Strong communication, interpersonal, and problem-solving skills
• Ability to work independently while contributing positively to a collaborative team
• Compassionate, patient-focused approach with good documentation practices

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
