Roles & Responsibilities

• Provide high-quality physical therapy evaluations and evidence-based treatment plans for outpatient and home-based patients
• Perform functional assessments, document findings thoroughly in Cerner, and complete OASIS forms as required
• Coordinate care with occupational therapy, speech therapy, rehab aides, OTA/PTA staff, and interdisciplinary teams
• Monitor patient progress, modify treatment plans, and communicate changes to managers and caregivers
• Educate patients and families on home exercise programs, safety, and discharge planning
• Participate in floor-specific orientation and regional assignments to ensure consistent coverage and quality care

Qualifications

• Active and unrestricted Physical Therapist license (state licensure required)
• Minimum 2 years of clinical PT experience (home health experience preferred)
• BLS certification required
• Experience with OASIS documentation (required)
• Proficiency with Cerner or similar EMR for accurate charting
• Comfort supervising and collaborating with OTA/PTA and rehab aides
• Willingness to work every 3rd weekend and provide occasional holiday coverage
• Strong communication and patient education skills

Knowledge & Skills

• Skilled in orthopedics, joint and spinal injury management, stroke rehabilitation, and general PT interventions
• Experienced in home health care workflows and regional patient coverage
• Proficient documentation skills, including completing OASIS and using Cerner
• Ability to create individualized, measurable treatment plans and outcomes
• Good clinical judgment, time management, and team collaboration
• Familiarity with safety protocols and delivering care in diverse home settings
