Roles & Responsibilities

Provide compassionate nursing care to clinic patients within the scope of LPN licensure, following established standards and practices
Serve as the primary liaison between patients, clinical staff, and providers to ensure coordinated, patient-centered care
Perform patient assessments including taking vital signs and documenting findings accurately
Triage patient calls, assess urgency, and escalate to providers or RN as appropriate
Manage prescription refills and complete necessary prior authorizations to keep patients' therapies on track
Support and direct CNAs and schedulers to maintain smooth clinic operations
Collaborate with providers, coordinators, clinical managers/supervisors, and RNs to meet patient care goals

Qualifications

Current State LPN license required
Graduate of an accredited nursing program
BLS (Basic Life Support) certification required
Comfortable performing triage, medication administration, and prior authorization tasks
Ability to give direction to CNAs and schedulers and receive direction from RNs and providers
Willingness to meet continuing education requirements as dictated by state and federal licensure

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Patient assessment and clinical documentation
Triage and telephone triage skills
Vital signs collection and interpretation
Medication administration and prescription refill workflows
Prior authorization processing
CNA supervision and teamwork with interdisciplinary teams
Strong communication and liaison abilities with patients and providers
