Roles & Responsibilities

Provide comprehensive outpatient primary care for patients across the lifespan with compassion and clinical excellence
Build long-term, trusting relationships with patients and families through longitudinal care
Diagnose, treat, and manage acute and chronic conditions in a community-focused practice
Collaborate closely with nurses, advanced practice clinicians, and administrative staff to deliver coordinated care
Participate in quality improvement and community health initiatives to improve local outcomes
Maintain accurate medical records and follow best practices for patient-centered documentation

Qualifications

MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine
Eligible for medical licensure in New York State
DEA certification or eligibility
Passion for serving a rural community and practicing patient-centered primary care

Education

MD or DO from an accredited medical school

Knowledge & Skills

Outpatient primary care and family medicine across the lifespan
Patient-centered, longitudinal care approach
Clinical team collaboration and interdisciplinary communication
Licensure and DEA compliance and related professional requirements
Community-focused healthcare and rural health needs
