Roles & Responsibilities

• Assess patients in the home setting and develop individualized treatment plans
• Deliver skilled therapeutic interventions to improve mobility, function, and safety
• Document timely, accurate clinical notes in the electronic health record (Epic preferred)
• Coordinate care with nurses, aides, physicians, and family members to achieve goals
• Educate patients and caregivers on home exercise programs, fall prevention, and safe transfers
• Perform functional outcome measures and modify treatment plans based on progress
• Participate in Trinity specialty competency testing and skills checklists to maintain clinical standards
• Ensure compliance with licensing, infection control, and required health screenings

Qualifications

• Current Physical Therapist license (must be valid for practice in Iowa)
• Basic Life Support (BLS) certification
• License/Certificate verification required prior to start
• Epic experience preferred but not required
• Completion of Trinity specialty competency testing and skills checklist prior to assignment
• Up-to-date immunizations and health screenings as required (MMR, Hep B, Varicella, Tdap, COVID-19, influenza, TB screening, drug screen, fit test)

Knowledge & Skills

• Strong clinical assessment and treatment planning for adult home health patients
• Skilled in therapeutic exercise, mobility training, and fall prevention strategies
• Comfortable working autonomously in patients' homes and adapting interventions to home environments
• Proficient documentation practices and familiarity with Epic charting workflows
• Excellent communication and care coordination with interdisciplinary teams and families
• Awareness of infection control practices and safety protocols in the home
