Roles & Responsibilities

• Collaborate with patients, families, nurses, physicians and the multidisciplinary team to design and implement individualized case management plans
• Serve as a trusted liaison and consultant to the healthcare team on patient-specific issues and care coordination
• Lead coordination of patient care across disciplines, ensuring timely and appropriate services
• Monitor diagnostic testing, treatments and procedures to align the care plan with clinical needs and covered services
• Partner with physicians to discuss resource utilization, consultations, estimated length of stay and discharge planning
• Focus on complex populations — frequent ED utilizers, chronic pain/substance use patients and people experiencing homelessness — to improve outcomes
• Coordinate transfers to tertiary centers and ensure continuity of care through accurate information transfer
• Collect and oversee data for regulatory and accreditation compliance, and conduct focus study reviews for frequent ED visitors

Qualifications

• 2+ years experience with a Medicare-certified home health agency (required)
• Travel assignment experience (required)
• Active, clear driver's license and current car insurance (required; documentation required)
• Valid Hawaii professional license (required; documentation required)
• Proficiency with Homecare Home Base and OASIS (required)
• COVID-19 and seasonal flu vaccinations required (no declinations accepted; documentation required)
• Bachelor's degree preferred
• Previous Case Manager experience preferred
• Strong communication, clinical judgement and interdisciplinary collaboration skills
