Roles & Responsibilities

• Deliver compassionate bedside care to residents in a skilled nursing facility
• Administer medications and treatments as prescribed by physicians
• Monitor vital signs, observe changes in condition, and report to RNs and providers
• Assist residents with activities of daily living and promote independence
• Document patient care accurately and timely in electronic health records
• Collaborate with interdisciplinary team to ensure quality outcomes

Qualifications

• Valid LPN license in the state of New Hampshire (or compact license)
• Current BLS certification from the American Heart Association
• Minimum of 1 year of recent LPN experience in long-term care or skilled nursing
• Ability to pass a background check and drug screening
• Commitment to providing patient-centered care with empathy and professionalism

Knowledge & Skills

• Proficient in nursing care procedures, infection control, and safety protocols
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently as well as part of a team
• Comfortable with electronic medical records and documentation systems
