Roles & Responsibilities

• Assess patients' functional mobility and develop individualized treatment plans that promote independence
• Deliver skilled physical therapy interventions (therapeutic exercise, gait training, manual therapy) to support recovery
• Collaborate with nursing and interdisciplinary teams to coordinate care and support safe discharges
• Document patient progress clearly and timely in the medical record, meeting SNF and payer requirements
• Educate patients and families on home programs, safety, and fall prevention to maximize outcomes
• Participate in team meetings and contribute to quality improvement initiatives that enhance resident care

Qualifications

• Current and unrestricted Physical Therapist license in Michigan, or eligible to obtain one prior to start
• Experience in Skilled Nursing Facility (SNF) settings preferred
• BLS/CPR certification (or willingness to obtain prior to start)
• Comfortable with documentation in electronic medical records and meeting SNF documentation standards
• Ability to travel and work on a contract assignment; reliable transportation and flexibility required
• Clear background check and up-to-date immunizations as required by facility

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s or Bachelor’s in Physical Therapy considered

Knowledge & Skills

• Skilled in patient assessment, therapeutic exercise, gait and balance training
• Knowledge of SNF care standards, Medicare documentation, and discharge planning
• Strong clinical reasoning and ability to prioritize caseloads
• Proficient communication and teamwork skills for interdisciplinary collaboration
• Familiarity with common assistive devices and safe transfer techniques
• Comfortable using EMR systems for documentation and billing support
