Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a 130-bed skilled nursing facility
• Assess functional status and develop individualized, outcome-driven treatment plans
• Implement therapeutic interventions to improve ADLs, mobility, cognition, and independence
• Collaborate closely with nurses, PTs, SLPs, and case managers to coordinate care
• Document progress and outcomes accurately and timely in the facility’s medical record
• Educate residents and families on strategies and adaptive equipment to maximize independence
• Participate in discharge planning and interdisciplinary team meetings to support smooth transitions

Qualifications

• Current California Occupational Therapy license (required)
• NBCOT certification preferred
• Skilled Nursing Facility experience preferred (SNF experience a plus)
• Current BLS/CPR certification or willingness to obtain
• Strong communication skills and ability to work well within an interdisciplinary team
• Willingness to participate in alternate weekend rotation

Education

• Bachelor’s or Master’s degree in Occupational Therapy (OT)

Knowledge & Skills

• Geriatric assessment and SNF-focused intervention strategies
• ADL and IADL training, adaptive equipment recommendation and training
• Cognitive and neuromuscular treatment approaches relevant to the SNF population
• Skilled documentation practices and familiarity with clinical charting
• Ability to collaborate effectively with clinical teams and families
