Roles & Responsibilities

• Perform diagnostic assessments to evaluate functional needs and goals for adult patients
• Develop and implement individualized occupational therapy treatment plans
• Provide professional OT services across the full range of rehab cases within the facility
• Deliver skilled rehabilitation interventions in a SNF/rehab setting to support patient independence
• Document assessments, plans and progress clearly in the EMR (Meditech) and communicate with the care team
• Collaborate with interdisciplinary team members to coordinate care and optimize outcomes
• Be available to work shifts, weekends and holidays as needed to support 24/7 services

Qualifications

• State of Hawaii Occupational Therapy license required by start of assignment (candidates not holding an active HI license must apply upon acceptance)
• BLS certification required
• Minimum 2 years work experience in at least one adult setting (Acute, SNF, rehab hospital, or outpatient)
• Experience working with adult patient populations and providing evidence-based rehab interventions
• Comfortable using Meditech EMR for documentation and care coordination
• Willingness to work a 5-day schedule with occasional weekend/holiday coverage

Knowledge & Skills

• Skilled in diagnostic assessments and treatment planning for rehabilitation
• Strong knowledge of SNF rehab principles and adult patient care
• Proficient documentation and clinical reasoning skills
• Familiarity with adaptive equipment and therapeutic modalities used in occupational therapy
• Effective communication and teamwork within interdisciplinary care settings
• Ability to prioritize caseload and manage time during busy departmental coverage
