Roles & Responsibilities

• Provide compassionate in-home nursing visits across private homes, assisted livings, senior apartments and board-and-care facilities
• Perform up to 6 patient visits per day delivering hands-on wound care, negative pressure wound therapy support, and PT INR testing in the home
• Accurately document patient encounters and care plans in HomeCare Homebase (HCHB) and complete OASIS documentation as required
• Coordinate with the Mercy care network and community providers to ensure continuity of care for patients transitioning from hospitals
• Educate patients and caregivers on treatment plans, wound care management, and in-home safety considerations
• Collaborate with your team and float when needed to meet patient needs across service territories

Qualifications

• Current and valid California LPN/LVN license (required)
• BLS certification (required)
• NPI number required
• Active CA driver’s license and proof of automobile insurance (required for HH role)
• Minimum 2 years experience in a healthcare setting (home health or community/home health experience preferred)
• Home Health experience including OASIS required; familiarity with HomeCare Homebase (HCHB) preferred
• Comfort working in varied home environments and able to adapt to cramped or cluttered spaces when necessary
• Two supervisory references at submittal (one within 12 months and one within 3 years) as noted in submission requirements
• Candidates not employed by CommonSpirit/CHI/Dignity facilities within the past year are eligible; PRN employment via outside firms is considered

Education

• Graduate of an accredited LPN/LVN program or equivalent vocational training
• Current California LPN/LVN license required

Knowledge & Skills

• Proficient charting and documentation practices, especially in HomeCare Homebase (HCHB)
• Strong wound care knowledge, including negative pressure wound vacuums (NPWT)
• Competency with PT INR testing performed in home settings
• Excellent oral and written communication skills
• Strong organizational and time-management skills to manage up to six home visits per day
• Ability to work independently in the field after orientation and precepting
