Roles & Responsibilities

• Provide skilled therapeutic interventions and assist with plan of care implementation for residents
• Perform gait training, therapeutic exercise, transfers and mobility training with safety and dignity
• Document patient progress promptly and accurately in the medical record
• Collaborate with PTs, nursing staff, and interdisciplinary team to optimize resident outcomes
• Educate residents and caregivers on home exercise programs, safety, and fall prevention
• Support discharge planning and communicate functional status to the care team

Qualifications

• Current and unrestricted PTA license in the state of Texas (or eligibility to obtain)
• Graduate of an accredited Physical Therapist Assistant program
• Basic Life Support (BLS/CPR) certification (current)
• Previous experience in skilled nursing, long-term care, or geriatrics preferred
• Strong communication skills and ability to collaborate with interdisciplinary teams
• Ability to pass background check and drug screen as required by the client

Education

• Associate's degree from an accredited Physical Therapist Assistant program (required)

Knowledge & Skills

• Experience with therapeutic exercise, modalities, and mobility training
• Skilled in transfers, gait training, and ADL-focused interventions
• Proficient documentation and care planning skills
• Knowledge of fall prevention strategies and safe patient handling
• Comfort working with an older adult population and complex medical conditions
