Roles & Responsibilities

• Conduct thorough in-home assessments to determine senior eligibility for the PACE program
• Guide seniors and families through the enrollment process with clarity and compassion
• Complete and maintain accurate enrollment paperwork and electronic records
• Coordinate with clinical teams, providers, and caregivers to gather necessary verifications
• Plan and execute travel within San Diego County safely and efficiently for assessments
• Communicate enrollment status and next steps to participants and internal teams
• Help improve enrollment processes by identifying documentation or workflow gaps

Qualifications

• Minimum 1 year of relevant experience
• Bachelor's degree OR 3 years of related experience (accepted in lieu of degree)
• Valid driver's license, reliable transportation, and proof of insurance required
• Experience with healthcare eligibility, in-home visits, or medical verifications preferred
• Strong verbal and written communication skills and excellent attention to detail
• Comfortable working independently in the field and handling confidential information

Education

• Bachelor's Degree OR 3 years of related experience in lieu of degree

Knowledge & Skills

• Familiarity with enrollment processes and eligibility determination
• Strong documentation and record-keeping skills (electronic and paper)
• Excellent interpersonal skills for working with seniors, families, and providers
• Time management and route planning for efficient in-county travel
• Basic computer skills: email, MS Office; experience with EMR or care management systems a plus
• Understanding of confidentiality/HIPAA principles and professional boundaries
