Roles & Responsibilities

• Assess patients' functional abilities and design individualized occupational therapy plans
• Develop and implement evidence-based interventions to maximize independence
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Document progress thoroughly and communicate outcomes with patients and families
• Educate patients, caregivers, and community members to support lasting functional gains
• Adapt treatment plans based on ongoing evaluation and measurable goals

Qualifications

• Licensed Occupational Therapist (OT) in Minnesota or eligible for MN licensure
• Graduation from an accredited Occupational Therapy program (entry-level OTD, MOT, or equivalent)
• Current basic life support (BLS) certification or willingness to obtain upon hire
• Comfort with outpatient rehabilitation settings and a passion for hands-on patient care
• Clear background check and ability to meet standard onboarding requirements
• Strong interpersonal skills and the ability to work independently while contributing to a team

Education

• Entry-level OTD, Master of Occupational Therapy (MOT), or equivalent from an accredited program

Knowledge & Skills

• Skilled in functional assessments and activity-based intervention planning
• Proficient with documentation standards and outcome measurement tools
• Excellent verbal and written communication with patients, families, and staff
• Time management and caseload prioritization in a busy outpatient clinic
• Familiarity with adaptive equipment, splinting, and therapeutic modalities
