Roles & Responsibilities

• Conduct screening, assessment, diagnosis, treatment planning, and brief therapy for patients from birth through elder
• Lead and coordinate discharge planning for ALF and transitional placements
• Facilitate or co-facilitate patient care conferences with multidisciplinary teams
• Provide care coordination and client advocacy, including assistance with complex issues such as Power of Attorney when relevant
• Support chemical dependency and mental health interventions and make appropriate referrals
• Maintain accurate, timely documentation to meet state, federal, accreditation, and hospital billing standards
• Complete provider enrollment and billing duties as assigned to support seamless revenue processes
• Participate in staff trainings and quality management activities to continuously improve patient care

Qualifications

• Master's degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed or in process of becoming Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
• Clinical Competency Assessment (SEARHC) required within 3 months of hire and every three years
• Valid State of Alaska driver’s license or obtainable within 6 months of hire
• Must be 21 years of age
• Demonstrated ability to complete required clinical documentation and meet hospital billing requirements

Education

• Master's Degree in Social Work, Psychology, Counseling, or related discipline (accredited institution)

Knowledge & Skills

• Discharge planning and care coordination across the continuum
• Screening, clinical assessment, and brief therapeutic interventions
• Interviewing techniques and crisis intervention skills
• Knowledge of ethical, legal management practices and patient rights
• Strong case management and clinical documentation skills
• Ability to observe, evaluate, and document psychiatric and substance use behaviors
• Comfort working in multidisciplinary teams and with tribal health/cultural wellness approaches
