Roles & Responsibilities

• Provide skilled speech-language pathology assessments and individualized treatment plans for SNF residents
• Deliver evidence-based therapy for speech, language, cognition and swallowing disorders
• Collaborate with nursing and interdisciplinary teams to support resident-centered goals
• Document progress and maintain timely, accurate clinical records per facility and CRG policies
• Meet facility productivity expectations and contribute to a high-functioning therapy department
• Participate in team meetings and discharge planning to ensure continuity of care

Qualifications

• Licensed Speech-Language Pathologist (SLP) or eligible to practice (verify state-specific licensure requirements)
• 1-3 years of clinical experience preferred (per job notes: 1-3 years)
• Comfortable working in a skilled nursing facility (SNF) / long-term care environment
• Willingness to follow CRG policies & procedures and meet a 90% productivity requirement
• Able to work on-site (therapist must be present in the building to receive guaranteed hours)
• Professional appearance in black scrubs as required by the facility
• Strong communication and teamwork skills with a resident-centered approach
