Roles & Responsibilities

• Provide comprehensive in-home physical therapy evaluations and treatments across varied diagnoses (ortho, cardio, pulmonary, neuro, geriatric, oncology, amputations)
• Perform Start of Care visits and ongoing patient evaluations, balancing travel, documentation, and case management
• Document OASIS and clinical notes accurately and timely using the EMR (HomeCare Homebase preferred)
• Coordinate care and communicate effectively with RN managers, physicians, and interdisciplinary teams to support patient outcomes
• Supervise and mentor up to two PTA/COTAs, ensuring quality and continuity of care
• Manage caseloads efficiently across assigned territory while maintaining patient-centered care in home environments
• Participate in regular meetings and sign timesheets as required to support administrative processes

Qualifications

• Current California Physical Therapy license (required)
• BLS (AHA) certification (required)
• NPI number (required)
• Minimum 2 years of experience in a healthcare setting; home health/community experience preferred
• Home Health experience with OASIS required; EMR/HomeCare Homebase (HCHB) experience preferred
• Valid driver's license and proof of automobile insurance required for home visits
• Strong oral and written communication skills and good organizational abilities
• Two supervisor references required at submittal (charge accepted): one within 12 months and one within 3 years
• Candidates must not have been directly employed by CommonSpirit (including CHI/Dignity) within the past year; PRN employment via outside firms is acceptable

Knowledge & Skills

• Patient evaluation and individualized treatment planning in the home setting
• OASIS documentation and home health regulatory knowledge
• Case management and efficient documentation practices
• Experience with Start of Care visits and weighted visit planning (travel + treatment + documentation)
• Familiarity with HomeCare Homebase (HCHB) or similar home health EMRs
• Ability to work in varied and sometimes challenging home environments
• Supervisory skills for overseeing PTA/COTA staff and coordinating workflows
